Vita10 Therapy (Vita10 IV Therapy, LLC and Total IV Solutions, PLLC) is committed to protecting the privacy of your health information. We are required by law to maintain the privacy of your protected health information (PHI), to provide you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

 

How We May Use and Disclose Your Health Information

    • Treatment: To provide, coordinate, and manage your care, including sharing information among your providers.
    • Payment: To bill and collect payment for the services you receive.
    • Health Care Operations: To support the business activities of our practice, such as quality improvement, training, and administration.
    • As Required by Law: When federal, state, or local law requires it.

Other uses and disclosures not described in this notice will be made only with your written authorization, which you may revoke at any time.

 

Your Rights

You have the right to: request access to and a copy of your health records; request corrections to your records; request restrictions on certain uses and disclosures; request confidential communications; receive an accounting of certain disclosures; and receive a paper copy of this notice. To exercise any of these rights, contact our Privacy Officer.

 

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
